1. Assumption of Risk
I, the undersigned, acknowledge that participation in fitness training, martial arts, exercise programs, and related activities offered by The Sweat Lab NC LLC involves inherent risks of physical injury, illness, disability, or death. These risks may arise from my own actions or inactions, the actions or inactions of others (including instructors, other participants, or staff), the condition of the facilities, or the nature of the activities themselves. I understand these risks may include, but are not limited to, muscle strains, sprains, fractures, heart attack, heat exhaustion, communicable diseases, and other potential injuries. I voluntarily assume full responsibility for all such risks associated with my participation.

2. Health Representation
I, the undersigned, acknowledge that participation in fitness training, martial arts, exercise programs, and related activities offered by The Sweat Lab LLC involves inherent risks of physical injury, illness, disability, or death. 3. Release of Liability & Covenant Not to Sue
In consideration for being permitted to participate in activities at The Sweat Lab LLC, I, for myself, my heirs, executors, administrators, successors, and assigns, hereby fully and forever release, waive, discharge, and covenant not to sue The Sweat Lab LLC, its owners, officers, directors, employees, instructors, agents, volunteers, and other participants (collectively "Releasees") from any and all liability, claims, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Releasees or otherwise, while participating in such activity, or while in, on or upon the premises where the activity is being conducted.

 By checking this box, I confirm I am 18+ years old, have read this entire agreement, fully understand its terms, understand that I have given up substantial rights, and agree to be bound by its terms freely and voluntarily. *

Subscription Payment Agreement
 I acknowledge and agree to the recurring payment terms for the selected membership subscription as displayed in the order summary. 

You are purchasing the subscription plan detailed in the order summary above. By proceeding, you authorize The Sweat Lab NC LLC to charge your selected payment method on a recurring basis according to the plan terms. Your first payment occurs upon checkout completion. Subsequent payments will be charged automatically unless canceled per the terms. You acknowledge the freeze policy (max 2/year, $25 fee for 3rd+) outlined previously.
